As the hub for the statewide Kinship program as well as the Aging and Disability Resource Navigation service, our newest initiative - CommuniCare - is the next strategic step to providing access to care through this statewide Community Information Exchange initiative. With a committed state partnership with Medicaid, we are excited to see the outcome of this new level of care.
