This session will present lessons learned from an 18-month collaborative effort of 11 local United Ways, including nine that operate 211 Information & Referral programs, in the digital transformation of their social care work, to enable data sharing with public and private health systems, closed-loop referral across County community care networks, and establish earned revenue for 211 and United Way programs.
